How do dermal fillers work?

Dermal fillers (or soft tissue fillers) can be used to plump lips, smooth wrinkles, and folds, and fill in the under-eye hollows known as tear troughs. They can also add volume to cheeks, reduce the appearance of pitted acne scars, and sculpt the jawline.

The face loses subcutaneous fat and muscle volume with age. For patients who are seeing the early stages of age-related volume loss, a “liquid facelift” that combines injectable fillers with a neuromodulator like Botox can restore a more youthful appearance, delaying the need for a plastic surgery procedure like facial fat transfer.

That said, dermal fillers will not provide the same level of facial rejuvenation as a cosmetic surgery like a facelift; the results are more subtle and more temporary.

Hyaluronic acid is the most popular FDA-approved filler ingredient because it’s so easy to dissolve in the event of a complication. Fillers can also be made of collagen, calcium hydroxylapatite or (Radiesse), and poly-L-lactic acid (Sculptra).

FREQUENTLY ASKED QUESTIONS

What happens during a dermal filler treatment?

First, you and your injector will discuss your desired result and the best type of filler to create it, as well as potential risks. This is a great time to ask how much filler you'll need and get a more specific cost estimate, to avoid sticker shock when you go pay your bill. 

Once you’ve worked out these details, here's what you can expect:

  • Your injector will clean the treatment area and apply a topical anesthetic if needed (though most fillers contain lidocaine already).
  • Then they'll slowly inject the filler with a needle or blunt-tipped cannula, gently massaging the area with their fingers to distribute it before continuing with more injections, for even distribution.
  • If it's your first time getting fillers (particularly lip fillers), your injector may be conservative about how much filler to use and suggest that you return once the swelling has gone down, to assess your result and see if you want more volume. In fact, if you're getting Sculptra, you can expect to need a series of multiple treatments over a few months, as new collagen develops.
  • Each injection only takes a few moments, and the entire cosmetic procedure takes under 20 minutes unless you're having an extensive treatment like a liquid facelift.
  • You’ll see more volume or smoother skin immediately.
  • When they're done, they may give you a small, light ice pack to hold against your skin to alleviate discomfort, tenderness, or immediate swelling or bruising. Just don't apply too much pressure.  

How is treatment with Radiesse performed?

Radiesse is injected under the skin in a quick procedure that can be done over the lunch hour.

How soon will I see results with Radiesse, and how long will they last?

Immediately upon injection, Radiesse helps smooth out lines and wrinkles.1,2,4,5
The natural-looking results last a year or more in many patients.6,7

What happens during a Revanesse treatment?

First, you’ll first discuss your goals with your injector. They will evaluate your facial anatomy and determine how to best use filler (and, possibly, complementary treatments like Botox) to achieve your desired results.

They will then clean the treatment area and apply a numbing cream unless the selected formula already includes lidocaine (as with Revanesse Versa+).

They’ll inject the filler in the "mid-to-deep dermis" of your skin, using an ultrafine needle or a microcannula, while gently massaging the area with their fingers, to smooth it out.

You’ll notice a difference immediately, though it may take a couple of weeks for the filler to settle completely so you can see your final results.

Is Revanesse safe?

Revanesse treatments are considered to be safe. The most common side effects are bruising, swelling, and redness at the injection side, but these issues tend to be minor and fade relatively quickly.

Infection and allergic reactions are possible but very rare.

How Many Treatments Are Required with Sculptra?

On average, a series of three treatment sessions over the course of a few months may be needed. The number of injections at each session will vary, depending on the degree of correction needed and the treatment plan determined by you and your specialist.

How Soon Will I See Results with Sculptra?

Sculptra is designed to give you noticeable results that emerge gradually and can last up to two years.* Don’t be disappointed if you don’t see immediate results. A few days after treatment, you may look as you did before treatment. This is normal and temporary. The Sculptra regimen delivers its full effects with an average of three treatment sessions over the course of a few months. Because results happen gradually, you’ll see subtle, yet noticeable, results in the reduction of facial wrinkles and folds by comparing images before treatment and a couple of months following treatment.†

Is Sculptra Safe? What Are the Side Effects?

Sculptra is FDA approved in the United States and has been used worldwide since 1999. Sculptra is made of a synthetic material called poly-L-lactic acid that is absorbed naturally by the body and has been used for years in dissolvable stitches.

Since Sculptra does not contain any human or animal components, no allergy testing is needed prior to treatments.

Side effects reported include injection site pain, redness, bruising, tenderness, itching, and lumps, bleeding, or swelling. Small bumps under the skin can sometimes be noticeable when pressing on the treated area. Larger lumps, with or without inflammation, have also been reported.

The types and duration of side effects may vary. If you are experiencing side effects, talk to your doctor.

Are dermal fillers better than Botox?
